Pulmonary tuberculosis (TB) remains a public health issue requiring comprehensive management. Rimbo Tengah Health Center recorded 40 TB cases in 2023, increasing to 47 cases in 2024,with 27 cases documented through September 2025. Monitoring obstacles also stem from delayed examination results and unreported cases from the regional public hospital. The purpose of this study is to analyze the TB case detection and monitoring program at Rimbo Tengah Health Center. This research is a descriptive qualitative with data collection techniques using in-depth interviews, observation, and documentation. Eight key informants were interviewed, including the Head of the health center, TB Program Manager, a nurse, a health analyst, a sanitarian, a treatment supervisor (PMO), a TB Patient, and representatives from the Health Office. Data analyzed with method triangulation based on the Context–Input–Process–Product (CIPP) evaluation model. The results indicate that the context aspect is supported by national policies and regional needs, despite facing public stigma and low screening awareness. Regarding input, helathcare personnel are available but overburdened with dual roles, limited access to Molecular Rapid Tests (TCM), and non-specific funding. The process aspect demonstrates that program implementation aligns with national guidelines, yet remains suboptimal due to human resource constraints. The profuct aspect reveals a relatively high treatment success rate, though case detection remains low. In conclusion, the TB program at Rimbo Tengah Health Center operates according to policy guidelines but falls short of reaching target yields. Active screening, patient mentoring and community empowerment are essential to support the 2030 TB Elimination goal.
